In this field-based Health and Social Services Coordinator role, you will coordinate the long-term care needs of patients within local communities, backed by a dynamic and elite team. This position allows a flexible work schedule as you tackle various challenges. Based in Oahu, HI, you will work remotely, spending about 75% of your time engaging with members in their homes or long-term care facilities. Our teams operate out of the downtown Honolulu area and cover the West Side, East Side, and North Shore regions. Adaptability, patience, and a willingness to face various situations will be key to your success.

Compensation is determined by several factors including local market rates, education, experience, and certifications. In addition to competitive pay, you will enjoy a comprehensive benefits package, incentive programs, equity stock purchase, and a 401k contribution, all subject to eligibility requirements. The hourly pay for this role ranges from $28.27 to $50.48, based on full-time employment, in compliance with minimum wage laws.
