Job Title : Home Health Registered Nurse Profession : Registered Nurse Specialty : Home Health Duration : 20 weeks Shift : Day Hours per Shift : 8 hours (08 : 00 - 17 : 00) Experience : Minimum of 1 year of skilled home health experience required, OASIS training and experience preferred License : Valid state nursing license required Certifications : BCLS required Must-Have : Skilled home care experience is necessary. Home health experience with OASIS training and experience is essential. IV skills and wound care experience are required. Experience with high risk diagnoses such as heart failure, COPD, myocardial infarction, pneumonia, stroke, and post-operative care is needed. Candidate must have a valid driver's license and a vehicle for home visits. COVID vaccination or a valid exemption form is required. Flu shot or declination is mandatory, with mask-wearing while working. Description : The role involves providing skilled home health care visits to patients. Patients will require various assessments and care, including cardiopulmonary assessments and basic wound care. Additional responsibilities include ABI compression dressing, wound vac dressing, Pleurx / Asept drain care, and foley catheter change and care. Candidates will perform staple and suture removal, glucometer use, pro-time machine use, and lab draw venipuncture. IV skills, PICC line or Midline care and dressing change, and SQ port access / deaccess are essential tasks. Typical home skilled visits range from 6 to 8 per day. Day and shift hours may vary depending on the needs of the home care office. Candidates must have recent patient care experience within the last 3 months Up-to-date skills are expected to cover cardiopulmonary assessments and various wound care procedures. Interdisciplinary support includes physical therapy, interpretation services, social services, and pharmacy. This position requires flexibility in scheduling, including occasional weekend shifts based on patient needs.
