Join a collaborative, therapist-led team at Avamere at Three Fountains, a 117-bed skilled nursing facility with a spacious therapy gym, kitchen, and access to a health club and swimming pool for therapy sessions. We offer in-house therapy services across Avamere facilities in Oregon and Washingtonno travel between buildings required.

Qualifications :
Graduate of an approved Physical Therapy Assistant PTA curriculumHold an active license to practice Physical Therapy by the State Licensing BoardWorking knowledge of Medicare and other payer sourcesFull knowledge of resident's rightsExudes professionalism in presentationAbility to multitaskMust be able to read, write, speak, understand, and communicate in EnglishCPR certificationKey Responsibilities :
Implements and executes on established patient treatment plan under the supervision of a licensed Physical TherapistCommunicates with patients, families, referring physicians, and other members of the team Provides timely written documentation per facility and department requirementsDevelops and enhances clinical and professional skills through knowledge and professional associationsProvides timely written documentation as required by insurance provider to meet licensing, certification, regulatory, reimbursement requirementsSupports and participates in department operations and developmentReports any changes in patient status or needs to supervising therapist on a timely basisAt Avamere, we believe in taking care of our employees.
