Home Health RN Job Details

Location : New Lenox, IL 60451 (Chicago area home health jobs)

Healthcare Setting : Home Health / Quality Assurance

Schedule : 8 : 00 AM – 4 : 30 PM, 8-hour day shifts

Hours : 40 hours per week, Monday through Friday

Contract Duration : 13 weeks (Start : 11 / 3 / 2025 – End : 2 / 2 / 2026)

Employment Type : Travel Registered Nurse

Estimated Weekly Salary : $1860 - $2000 per week

Registered Nurse Qualifications

Minimum 2 years of Home Health RN experience (homecare nursing experience required)

Previous experience in Quality Assurance or clinical quality improvement

COS-C (Certificate for OASIS Specialist-Clinical) Certification

OASIS experience required for home health RN jobs

Active Illinois Registered Nurse license

Valid driver’s license and car insurance (required for travel nursing jobs)

Current CPR certification

COVID-19 vaccination required (declinations not accepted)

Submission of resume, skills checklist (within 1 year), 2 supervisory references, active certifications, and license required

Homecare RN Responsibilities

Implement and evaluate performance improvement, quality, and utilization activities for home health services

Conduct reviews of medical records to ensure quality and compliance with healthcare standards

Facilitate interdisciplinary case conferences and monitor coordination of care and OASIS data

Monitor OASIS data integrity and complete timely OASIS data submission

Ensure development, implementation, and updates of individualized patient plans of care

Produce monthly reports to monitor quality of services delivered and publicly reportable data

Assist with evaluation of clinical competence and compliance for home health staff

Stay current on home health regulations, rules, and industry standards

Promote a clean and safe environment of care, utilizing SAFE error prevention habits

Maintain the highest standard of privacy and confidentiality in all matters involving patients and coworkers
