Remote Care Navigator – Senior & Dementia Care
We’re seeking a compassionate and experienced Care Navigator to join a mission-driven healthcare team supporting family caregivers nationwide. This fully remote position focuses on empowering those caring for seniors and individuals living with dementia through education, coordination, and emotional support.

- Collaborate with care teams, social workers, and case managers to maintain alignment
