Job Overview

Mixed Staffing and Recruiting is seeking a compassionate, reliable, and self-motivated Home Health Care Nurse to join our growing team. Our mission is to support a collaborative and diverse workforce where dedication, initiative, and quality care drive excellence.

In this role, you will provide individualized nursing care to patients in their homes, ensuring comfort, safety, and high-quality medical support in a familiar environment.

Key Responsibilities

Provide personalized nursing care to patients in their homes
Assess patient health conditions and monitor vital signs
Administer medications and treatments as prescribed
Collaborate with physicians and healthcare team members to develop and implement individualized care plans
Educate patients and families on medications, chronic conditions, and overall health management
Perform wound care, rehabilitation support, and other specialized nursing tasks as needed
Assist with tracheostomy care and suctioning (as required)
Manage G-tube feedings (as required)
Administer oxygen therapy (as required)
Provide daily basic patient care, including repositioning and mobility assistance
Complete accurate and timely documentation using electronic charting systems
Offer emotional support to patients and their families
Perform other duties as assigned

Qualifications & Requirements

Valid LPN or RN license
Knowledge of home health safety, sanitation standards, and procedures
Understanding of confidentiality, privacy laws (HIPAA), and nursing best practices
Strong understanding of medical terminology
Ability to learn and use electronic verification/charting software
Ability to safely turn and reposition patients
Excellent communication and interpersonal skills
Compassionate, dependable, and able to handle challenging situations professionally
2+ years of nursing experience preferred (not required)
