Description : Position Summary :
Under general direction, has authority, responsibility, and accountability for assessment, planning, implementation and evaluation of wounds, ostomy, and continence care throughout the agency. Functions primarily as a consultant; provides direct patient care and education to selected patients. Provides staff development in areas of expertise. Responsible for product recommendations in areas of expertise. Maintains records and statistics reflecting role performance; participates in agency wide committees and projects as indicated. Participates in wound telemedicine program.

Essential Duties & Responsibilities :
- Updates and implements patient plan of care.
- Assesses patients physiological, psychosocial, environmental and spiritual needs.
- Demonstrates physical assessment skills.
- Administers medications.
- Applies critical thinking skills when delegating / assigning tasks.
- Works with coordinator and other licensed personnel to assist in developing a plan of care.
- Provides an environment conducive to safety for patients, visitors and staff. Complies with appropriate and approved safety standards.
- Communicates with patients, family, care givers, significant others and members of the health care team to promote maximum benefit of care.
- Utilizes the appropriate leadership skills in delegating, organizing and education of coworkers and staff.
- Demonstrates an understanding of costs as they relate to quality and efficiency.
- Provides patient care and consultation to staff regarding management of patients with :
- Ostomy or continent diversion.
- Potential or actual alteration in skin integrity.
- Percutaneous tubes.
- Urinary and fecal incontinence.
- Assist staff in maintaining current knowledge and competence in the areas of wound, ostomy and incontinence.
- Conducts product evaluations and submits reports and recommendations.
- Provides enteral consultation to patient and staff in local and extended areas. Documents patient records according to agency policy. Completes clinical notes, Plan of Care and medication records in a timely, legible and accurate manner. Documents care conferences reflecting patients progress towards goals.
- Operates medical equipment correctly for patient care.
- Maintains current knowledge of nursing and home care agencys policies and procedures.
- Participates in the PI program by utilizing suggestion boxes, customer service, and brings matters to attention of management staff.

- Understands the infrastructure and how and where to transfer calls within the company.

- Participates in accreditation program.
- Participates in back up call according to the needs of the agency.
- Submits all clinical documentation in a timely manner according to agency policy.
- Performs other duties as assigned.
Education / Experience :
Two years nursing experience within last four years.Work experience denoting communication and assessment skills.Graduate of an accredited BSN program required.Licenses, Certifications and / or Registrations :
Current RN license in the State of Florida.Graduate from an accredited WOC Nursing program, Board Certification.Current CPR certification.Current Driver License and valid auto insurance.HIV / AIDS / Domestic Violence education required annually.Working Conditions :
