RN / LCSW Case Manager

A-Line is hiring Full-Time Registerer Nurse Case Managers - LTC with a Major Health Plan company based in Greensboro, NC OR Guilford County, NC.

• This is a Hybrid position doing 3-4 days of visits in LTC facilities with 1-2 days remote! The RN or LCSW must reside in Guilford County, NC!

Hourly Rate : $40.00-42.00 PLUS Mileage reimbursement & Lodging reimbursement if needed

Work Schedule : Monday–Friday, 8 AM – 5 PM EST

Position Purpose

The LTSS (Long-Term Services and Supports) Service Care Manager is responsible for assessing, developing, and coordinating holistic care management activities for members with long-term care needs. This role focuses on improving quality outcomes, ensuring cost-effective care, and supporting members and families through education and coordination of available services.

This position is member-facing and may also include interaction with providers and community resources.
