Job descriptionOur management team is very involved & uses a hand-on approach to help you succeed. Gain confidence & a sense of self-fulfillment, while having a positive impact serving others. Long-term care is a very rewarding experience. Provide a comprehensive physical therapy evaluation based on MD's orders. Document findings on the standardized evaluation form. Provide a comprehensive treatment plan, including : long- and short-term goals, frequency, duration, and treatment modalities. Ensure MD orders are obtained for evaluations, treatments, re-certifications, and discharges. Provide comprehensive treatment to patients utilizing modalities and modify patient treatment as indicated while adhering to precautions. Complete any and all documentation forms required by Bedrock Care. Graduate of an accredited university with an MA or MS in communication disorders that is recognized by ASHA and state board. Holds current license and / or registration in the state of Pennsylvania. Successful completion of CFY.
