WAGE RANGE : $24.00 – $26.50 / hour
JOB SUMMARY :
Catholic Charities is seeking a self-motivated advocate who is compassionate, trauma-informed, and culturally aware to join our Housing Focused Behavioral Support Team (HBST) at our St. Paul Opportunity Center / Higher Ground - Minneapolis locations.
This position provides direct case management to adults with serious mental illness and co-occurring conditions who are experiencing homelessness or housing instability. The Case Manager II works alongside clients to assess needs, develop individualized service plans, access resources, and strengthen skills for long-term stability.
They serve as both advocate and connector - helping clients navigate complex systems of care while promoting dignity, safety, and independence. The role collaborates closely with internal teams and community partners to ensure clients receive consistent, trauma-informed support.
HBST provides flexible services across a range of care settings - from Housing Stabilization Services and wraparound case management to brief interventions supporting clients with existing service connections. The team works in close partnership with emergency responders, housing providers, and other Catholic Charities programs to ensure clients receive coordinated, compassionate care.
This position requires a valid Minnesota driver’s license, a reliable vehicle, and proof of insurance.
ESSENTIAL FUNCTIONS
MINIMUM QUALIFICATIONS / EXPERTISE
In lieu of degree, three (3) years of directly related experience required.
