Job description

Become a vital part of ACE’s Community Violence Intervention (CVI) team as the Lead Case Manager, where you'll provide technical support to a team of Care Managers and ensure high-quality services for participants. In collaboration with the Lead Community Intervention Worker (CIW), you’ll oversee care management services following the Family Case Management (FCM) Model and Transitional Client Services (TCS) Model, maintaining alignment with CVI policies. As a community liaison, you’ll build partnerships with organizations serving CVI clients, fostering essential connections.

ACE’s CVI program serves individuals aged 15 to adult, who are gang-affected or otherwise at risk of gang involvement. Through outreach activities and incident response, the program promotes community-level peacemaking and violence reduction. Through a phased program of participant engagement, potentially leading to cycles of family case management services, the program works to strengthen participants' family ties and other pro-social influences.

DUTIES AND RESPONSIBILITIES :

  • Coordinate and perform outreach and marketing duties to recruit gang-impacted participants and educate the community on ACE services.
  • Coordinate intake and eligibility determinations and make appropriate referrals when the applicant is not eligible for ACE services; conduct participant assessments leading to enrollment; complete and update participant service plans.
  • Provide case management and care coordination for an average standing caseload of 12 participants with complex health and / or social needs, including developing individual care plans, arranging / participating family and service team meetings, and coordinating wrap-around services with other service providers.
  • Provide the necessary resources and skills to assist participants involved in community violence situations.
  • Advocate on behalf of participants with healthcare providers to ensure proper care and coordinate transitions between healthcare settings and social service providers, as needed.
  • Partake in mediation and advocacy as necessary, conducting home visits in high-crime areas, hostile environments, courts, and with their probation officers.
  • Provide safe passage across gang lines to gang-impacted participants to ensure the safety of participants to receive necessary services.
  • Organize, coordinate, prepare, and deliver skills-based workshops for gang-impacted participants and their families.
  • Organize celebration activity meetings with participants and support systems.
  • Collaborate with FCHN to align care coordination efforts, regular reporting on health and social outcomes, and share participant progress.
  • Coordinate Care Managers for the proper delivery of services, which will include the allocation of caseloads and service need determinations.
  • Provide technical support for Care Managers (e.g. provides care managers with strategies for effective case management service delivery).
  • Coordinate with Lead CIW for services to CVI clients.
  • Assist the CVI Manager - Case Management in the monitoring of daily CVI Care Management activities and report any pertinent information to them.
  • Assists and works closely with Lead Community Intervention Worker (CIW), and CVI Program Managers.
  • Foster positive relationships with community members, partner organizations, and local gangs.
  • Participate in all CVI community-based events, including Saturday Night Lights and Fall Friday Night activities, and in other cultural and community-building activities / events.
  • Model and mentor ACE’s best practices and core values to the collective CVI team.
  • Work to incorporate into organizational culture best practices for diversity, equity, and inclusion, restorative justice, and trauma-informed care.
  • Document all interactions and ensure compliance with funders / ACE / HIPAA regulations.
  • Assist with programmatic data audits and monitoring across multiple databases.
  • Other duties as assigned.

KNOWLEDGE, SKILLS, AND ABILITIES :

  • Is sensitive to the community, cultural, and personal dignity of program participants and is able to successfully engage, assess, and plan services for participants typical of the program’s target population.
  • Recognizes appropriate boundaries and limitations on interactions with program participants and their families, and the need to maintain professional standards and practices.
  • Is familiar with standard approaches to accurate participant record keeping, including timely charting, notation formats, content limitations, and confidentiality practices.
  • Possesses necessary de-escalation skills to provide crisis and community violence intervention in the community.
  • Possesses knowledge of programs, regulations, and procedures regarding human services, social services, justice, and workforce development.
  • Knowledge of community / gang dynamics within the San Fernando Valley.
  • Able to operate within the San Fernando Valley, exposing oneself to gangs found within the coverage area.
  • Able to work in assigned locations that may expose oneself to hazardous conditions or in extreme physical or discomfort or distress.
  • Ability to assess conditions and responsiveness to interventions.
  • Possesses understanding of Enhanced Care Management principles, including frequent participant contact, care plan development, and collaboration with Medi-Cal Managed Care Organizations (MCOs) and community-based organizations.
  • Understands complex care management principles and can develop care plans for participants with chronic health conditions and social needs. Demonstrates strong care management skills, including monitoring care plan adherence and coordinating multidisciplinary services.
  • Understanding of Medi-Cal systems and the social determinants of health affecting underserved populations is beneficial, with a willingness to build expertise in these areas.
  • Proficiency in navigating and using technology.
  • PHYSICAL REQUIREMENTS :

  • Prolonged periods standing and walking throughout facilities.
  • Prolonged periods of sitting at a desk and working on a computer.
  • Must be able to lift up to 15 pounds at times.
  • Must be able to bend, lift, stretch, climb, and crawl to provide services.
  • QUALIFICATIONS :

  • Bachelor’s Degree in a relevant field and 5+ years of gang intervention experience; completed Community Violence Intervention training, OR;
  • Equivalent combination of training and experience.
  • Spanish language skills, verbal and written, are highly preferred.
  • Possess a current California Driver’s License, clean DMV record & adequate auto insurance.
  • Must submit to a Department of Justice Criminal background check.
  • Initial TB employment clearance required.

  • The anticipated pay range for candidates who will work in California is $30.73 to $31.98 hourly The offered pay to a successful candidate will be dependent on several factors that may include but are not limited to years of experience within the job, years of experience within the required industry, education, etc.
