Location : LIFE at New Jersey

Shift : 1st Full Time

Hours : 8-4 : 30 Monday - Friday

Position Purpose :

This primary purpose of this position is to coordinate and schedule external appointments participants according to participant needs and care plan designed by the interdisciplinary team. Will work closely with the transportation department, participants, caregivers, and center staff.
