Home-Based Primary Care Opportunity in Chattanooga, Tennessee
Lead a collaborative care team and deliver personalized medicine directly to patients in their homes. This Physician Pod Leader role offers a rare blend of clinical autonomy, leadership, and a sustainable Monday-Friday schedule, all within a mission-focused, tech-enabled organization. Make a meaningful difference in the lives of Chattanooga's most vulnerable patients while guiding a tight-knit care team.
Practice Overview
- You'll lead a Chattanooga-based team of 2 APPs and manage a personal panel of ~140 patients
- 100% home-based care with no clinic or hospital responsibilities
- Blend of chronic disease management and coordinated team care in patients' homes
- Serve as clinical lead : mentoring, chart review, oversight, and quality improvement
