Schedule : Full -Time / Day

Shift : Day Shift Monday-Friday 8am to 5pm with e / o weekend or call rotation.

Full-time nurse position : Cicero Berwyn, Riverside, North Riverside, Stickney, Forest View, Lyons, McCook, surrounding towns

Full-time nurse position : North Naperville, Warrenville, Wheaton, Glen Ellen, Glendale Heights, Bloomingdale, Hanover Park, Schaumberg surrounding towns

The role you'll contribute :

his role involves providing direct nursing care in accordance with established policies, procedures, and protocols of the healthcare organization.

Under general supervision, the position is responsible for assessing, planning, implementing, and evaluating total patient care, as well as coordinating patient and family teaching.

This role assures compliance with nursing standards, as well as federal, state, and local government regulations, and organizational clinical policies.

It involves directing Licensed Practical Nurses and Home Health Aides to ensure quality care is provided according to physician orders under a patient care plan.

This position is accountable for the planning and implementation of patient care activities, coordinating the efforts of team members to achieve individual patient care plans, thereby maximizing function within the home, restoring health, or facilitating a peaceful death.

The role provides, coordinates, supervises, and responsibly directs the activities of various levels of assigned nurses and ancillary personnel in the provision of patient care, utilizing both professional and supervisory discretion and independent judgment.

Additionally, the role may require being on call for after-hours work to provide goal-directed care leading to specific outcomes, coordinating, and directing the home care patient's care based on individual patient needs.

  • Implements and monitors patient care plans.
  • Monitors, records and communicates patient condition as appropriate.

  • Serves as a primary coordinator of all disciplines for well-coordinated patient care.
  • Notes and carries out physician and nursing orders.
  • Assesses and coordinates patient's discharge planning needs with members of the healthcare team.
  • Qualifications

    The expertise and experiences you'll need to succeed :

  • Diploma from an accredited school / college of nursing required.
  • Work experience not required
  • BLS Provider credentialed from the American Heart Association (AHA) obtained prior to hire date or job transfer date.
  • Drivers License credentialed from the Illinois Department of Motor Vehicles (IL-DMV) obtained prior to hire date or job transfer date.
  • Registered Nurse credentialed from the Illinois Board of Nursing obtained prior to hire date or job transfer date.
