RN Case Manager - Home Health Agency | Part Time

Home Care Network (HCN) is seeking a highly skilled, experienced RN Case Manager to join our home health agency and serve existing and new clients across our Southeast Ohio region, primarily serving Jackson, Vinton, and Gallia Counties.

More about the role :

• This is a Part-Time, Hourly position, with the potential to grow into a Full-Time, Salary position (if desired, not required).
• Typical travel distances include a 30-minute radius daily + up to 60-minutes on occasion.
• Door-to-Door mileage reimbursement provided.
• Services include starts of care (SOC), recertifications, resumptions of care (ROC), and other assessments and services, as well as new hire orientations and reviews.
• Preferred experience includes 1+ years of experience in : home health agency, wound care, lab draws, and Oasis assessments.
