Salus Hospice is urgently seeking a compassionate and skilled Hospice RN Case Manager to join our dedicated team serving the Los Angeles County region.

This position will cover South Bay, Central Los Angeles, and West Los Angeles areas! Ideal candidate will live in Central Los Angeles.

Hospice RN Case Manager Responsibilities:


* Perform RN duties on hospice team.

* Serve as a RN Case Manager for the hospice Interdisciplinary Team

* Assess for signs and symptoms of discomfort including presence of pain and takes prompt and appropriate action

* Establishes plan of care while collaborating and communicating with the Interdisciplinary Team including physician when appropriate

* Coordinates all hospice services provided to patients/families in all settings to ensure quality of care

Ideal Hospice RN Case Manager Candidate Background:


* Registered Nurse with valid CA state license

* Minimum one year of hospice RN experience

* Strong background in medical/surgical, ICU/CCU, or ER is helpful

* Valid driver’s license, auto insurance, reliable transportation and willingness to drive to patients place of residence

* Must have excellent interpersonal skills and a passion for hospice

* Understanding of hospice philosophy and needs of the terminally ill

* Self-directed with ability to work with little supervision

* Current Physical and TB test results
