Position Overview

We’re seeking a compassionate and reliable Licensed Practical Nurse (LPN) to join our care team for the 3–11 PM shift , including every other weekend . This role is ideal for a nurse who thrives in a fast-paced, resident-centered environment and values teamwork, consistency, and quality care.

Key Responsibilities

Provide direct nursing care to residents under the supervision of an RN or physician

Administer medications and treatments as prescribed

Monitor and assess residents’ conditions and report changes

Assist in developing and implementing individualized care plans

Maintain accurate documentation and patient records

Support and guide CNAs and other support staff

Ensure compliance with facility policies and state regulations

Qualifications

Current, valid LPN license in the state of Delaware

Experience in long-term care or skilled nursing preferred

Strong clinical skills and attention to detail

Excellent communication and teamwork abilities

Ability to work every other weekend as scheduled
