This RN role focuses on assessment, care planning, disease prevention, and care coordination for patients in home and community settings.\n\nResponsibilities & Role Overview\n\n
• Perform health assessments and implement individualized care plans\n\n
• Provide care management and coordinate services with other providers\n\n
• Monitor and manage chronic, acute, and preventative health conditions\n\n
• Ensure physician communication, medication administration, and follow-up care\n\n
• Supervise LPNs and unlicensed staff as needed\n\n
• Provide training on health / safety protocols and individual-specific needs\n\n
• Support documentation including progress notes, health status updates, and required nursing paperwork\n\n
• Communicate with inpatient teams during admissions and discharges\n\n
• Promote quality, compliance, and infection control standards\n\n? Requirements\n\n
• Active RN license in Indiana or compact state\n\n
• Home health experience preferred\n\n
• OASIS experience preferred\n\n
• AHA BLS certification\n\n
• Valid driver's license and reliable transportation\n\n
• Must be comfortable with community-based and field care\n\n
