Job Summary
Partners In Home Care is seeking a dedicated, compassionate, and skilled Physical Therapist to join our Home Health team. In this role, youll provide evaluation, treatment, and rehabilitation services to patients in their homes while helping them regain strength, mobility, and independence. Youll work closely with an interdisciplinary team to ensure high-quality, evidence-based outcomes and patient-centered care.
Essential Duties and Responsibilities
Conduct comprehensive evaluations and develop individualized care plans for patients in the home setting.Provide skilled, evidence-based physical therapy interventions as prescribed by the physician and outlined in the plan of care.Adjust treatment frequency and duration based on patient progress and ongoing assessment.Complete and score OASIS assessments accurately, collaborating with the Quality team to ensure compliance.Maintain current knowledge of Medicare regulations, payer requirements, and Conditions of Participation.Perform therapeutic interventions, including wound care when appropriate.Document care and communication accurately using point-of-care systems.Coordinate daily schedule with the scheduler and optimize travel routes to support efficient, consistent care.Participate in interdisciplinary care conferences and agency quality improvement initiatives.Supervise Physical Therapist Assistants (PTAs) and CNAs as required.Manage patient care throughout the episode, including admissions, re-evaluations, and discharges.Maintain active Montana PT licensure and complete required continuing education.Uphold ethical and professional standards in all patient interactions and agency activities.Participate in agency training sessions and professional development opportunities.Minimum Qualifications
Bachelors degree or higher from an accredited Physical Therapy programCurrent licensure to practice Physical Therapy in the State of MontanaValid drivers license, reliable transportation, and appropriate auto insuranceAccess to high-speed internetTwo or more years of experience preferred; home health or clinical rehab experience is a plusSchedule
Typical workdays are Monday through Friday, with flexibility based on FTE status and patient care needs, as directed by the supervisor.
