Registered Nurse – Case Manager Position : RN Case Manager Location : Clinton, MD Shift : Night 3x12 (19 : 00–07 : 00), Every Other Weekend Start Date : 11 / 17 / 2025 Requirement : Minimum 2 years acute case management experience, prior travel preferred Certifications : BLS, Active MD RN License Skills & Experience :

• Coordinate patient care from admission through discharge in an acute care setting
• Perform utilization review using InterQual and Milliman guidelines
• Manage discharge planning and care transitions
• Collaborate with interdisciplinary teams to achieve safe and efficient outcomes
• Maintain accurate documentation within the EMR
• Provide education and support to patients and families
