Purpose :
Case Manager on the Community Support Individual Team (CSI) will coordinate and provide comprehensive recovery and rehabilitation services to consumers participating in CSI program. Interventions include a range of therapeutic and behavioral services for individuals with both severe mental illnesses and / or substance abuse disorders. Services are generally delivered in the community. Case Manager will provide services in a culturally sensitive and appropriate manner.
Minimum Qualifications :
- High School Diploma with at least 5 years of case management experience.
- Knowledge of Recovery principles, Co-occurring Disorders Treatment concepts, and / or Evidence Based Treatment Models preferred
- Excellent interpersonal, verbal, and written communication skills.
- Valid Illinois driver’s license is required.

Provide case management, outreach, and aftercare servicesAssess clients’ functioning, strengths, needs, and diagnosisAssess for indicators of abuseDevelop treatment plan collaboratively with client by defining goals, specific outcomes, and time frames for treatmentApply appropriate behavior management techniquesProvide culturally appropriate, competent and individualized linkage plans in accordance with each client’s age, gender and gender identity, sexual orientation, race, ethnicity, religious beliefs and cultureDetermine appropriate type and level of services utilizing the Level of Care Utilization System for Psychiatric and Addiction Services (LOCUS) developed by the American Association of Community PsychiatristFacilitate psychiatric hospitalization when indicatedWork cooperatively with other members of the treatment team, offering and accepting assistance when needed.Maintain positive and mutually respectful relationships with other human service providers, entitlement agencies, health care vendors, and the communityAssist with outcome measuresMaintain compliance at all times with clinical documentation, timelines and agency policies and procedures including meeting required timeframes for documentation and service notesReport to assigned supervisor and actively seek consultation whenever necessary or requested by supervisorDevelop close working relationships with DHS and DMH funded agencies, and other mental health service providers throughout the north and west sides of Chicago and suburbsMaintain compliance with confidentiality policies and procedures at all timesMaintain positive and mutually respectful relationships with other human service providers, entitlement agencies, health care vendors, and the communityOther duties as assigned by supervisor
