Certifications : Preferred – LCSW (Licensed Clinical Social Worker) or LPC (Licensed Practical Counselor)

Must-Have : Must be computer literate

Description :

Provides skilled interventions for the support and resolution of patient and family crises.

Facilitates problem-solving and decision-making.

Engages in advocacy and facilitation necessitated by life-changing events such as adoption, guardianship, abuse, placement, or bereavement.

Mediates risk factors and helps cope with disease or disability.

Addresses psychosocial and socioeconomic issues while adhering to departmental policies and procedures.

Receives referrals and assesses patients and families in a timely manner.

Integrates social work plan into overall patient care through interdisciplinary collaboration.

Provides skilled social work services in comprehensive case management, discharge planning, continuing care services, advocacy, and clinical social work services, including crisis intervention and patient / family education.

Addresses issues related to abuse and neglect and advanced directives.

Facilitates psychiatric placements and discusses end-of-life issues pertinent to the assigned patient population.

Utilizes hospital and community-based resources and entitlements effectively.

Refers patients, families, and hospital staff to appropriate services to ensure continuity and quality of care.

Develops specialized knowledge of resources related to the needs of specific patient populations.

Facilitates efforts to obtain insurance coverage for hospital and community-based services.

Documents the patient care plan, staff interventions, and outcomes promptly and completely in the medical record.

Completes statistical reports as required.

Collaborates with the interdisciplinary team to develop complex discharge plans and conducts transitional planning for patients moving to nursing homes and other facilities.

Continues professional development through supervision and attendance at conferences and mandatory training.

Encourages self-awareness about special issues related to cultural, religious, or sexual orientation sensitivity.

Participates in community education and support group activities.

Exercises independent judgment in making decisions from diverse alternatives that are subject to review in final stages.

Coordinates activities of others and works directly with patients, customers, and healthcare professionals.

Interacts frequently with individuals at a director level or above.
