• Provide nursing care and support to program participants, including health assessments, treatment planning, and coordination of medical services.\n
• Collaborate with the ACT team to ensure participants receive integrated physical and mental health care.\n\nResponsibilities : \n
• Conduct nursing assessments and develop individualized care plans\n
• Implement and evaluate nursing care\n
• Coordinate health services with the ACT team and other providers\n
• Lead group sessions on health, wellness, and recovery\n
• Educate staff on medical care and medication procedures\n
• Advocate for participants to access medical care and community supports\n
• Supervise LPNs on medication management\n
• Ensure participants have primary care providers and receive regular exams\n
• Administer medications, vaccines, and perform basic procedures\n
• Maintain accurate health records and documentation\n
• Provide culturally competent care and crisis intervention as needed\n\nQualifications : \n
• Licensed and registered as a professional nurse in NYS\n
• BSN preferred\n
• Valid driver's license\n
• Bilingual (Spanish) desirable\n
• Experience with serious mental illness, forensic populations, co-occurring disorders, or HIV / AIDS preferred\n
