Hours per Shift : 32 hours guaranteed

Experience : New graduates are welcome; SNF experience preferred

License : Current state license required

Certifications : Valid CPR certification preferred

Must-Have :

Candidates must ensure the prescreen is filled out and attached upon submission.

A resume and license verification also need to be attached.

If these items are not completed and attached, the candidate will be declined.

Candidates are required to have a COVID test 72 hours prior to the start date, with results issued 48 hours prior to the start date.

Description :

Candidates should review the credentialing checklist prior to submission :

State license verification is required.

A resume must be included.

Three references are necessary.

A county criminal background check for the last seven years for all counties lived and worked is needed.

A federal criminal background check for the last seven years or more is required.

A multi-state criminal search for the last seven years or more is necessary.

A nationwide sex offender search for the last seven years or more is required.

An HHS / OIG search is needed.

A GSA search is required.

A social security number trace must be included.

A two-step PPD or Quantiferon test or proof of a positive PPD and chest X-ray is required.

A SAM search must be completed.

An Oregon state background check is also necessary.

Position urgency is normal.

Holiday coverage is not required.
