• Monday / Wednesday / Friday : 7am - 4pm, Tuesday / Thursday : 9am - 6pm
• Patients age 2+ years
• Experience in routine wellness care and chronic care management
• Hospital privileges available
• Major holidays off including Thanksgiving and Christmas
• Independent family practice experience required
• Urgent care experience required
