The Hospital Liaison serves as the primary point of contact between Health Care Connectors and local hospitals, healthcare facilities, and referral sources. This role focuses on building and maintaining relationships with hospital staff, coordinating patient referrals, and facilitating smooth transitions from hospital care to home health or hospice services.
The Hospital Liaison works closely with clinical teams, discharge planners, and case managers to ensure patients receive timely and appropriate care after hospital discharge.
