Job description

Overview

Outer Cape Health Services is hiring a Quality Support Specialist to assist with outreach to engage patients in care and drive quality metrics under the direction of the Quality Manager and Population Health Director.

Responsibilities

  • Conduct telephone outreach in a professional manner.
  • Identify patients with care gaps via external databases or EHR reports.
  • Explain outreach purpose to patients or patient representatives for re-engagement of members overdue for appointments or preventative care gaps.
  • Schedule appointments and assist with previsit planning tasks (lab reminders, HCC coding cues, vaccine opportunities) to close quality care gaps.
  • Collaborate with the Quality Manager, Practice Managers, and Operations Coordinator to manage patient panels and facilitate new member engagement; notify staff when patients transfer care externally.
  • Outreach to patients via telephone, MyChart messages, and mailed letters promoting population health recommendations based on quality initiatives and clinical guidelines.
  • Attend and actively participate in team meetings (on-site or virtual).
  • Maintain patient confidentiality and follow OCHS policies and HIPAA guidelines.
  • Provide excellent customer service with a professional and responsive approach to patients and coworkers.
  • Verify patient demographics, active health insurance, and current PCP; ensure the EHR reflects this information.
  • Direct patient needs and clinical questions to appropriate staff members.
  • Complete data entry required from external provider sources for quality metric closure.
  • Request medical records when appropriate for quality metric gap closure.
  • Assist with Hierarchical Condition Coding (HCC) tasks for overview building on patient Problem Lists.
  • Support monthly Quality Oversight Board meeting minutes.
  • Other related duties and projects as assigned.

Qualifications

  • Associate's Degree required; Bachelor's degree preferred.
  • A minimum of 2 years' experience in a medical / clinical office environment; or prior experience as an LPN or Medical Assistant.
  • Knowledge of clinical patient registration and electronic medical record systems.
  • EPIC knowledge preferred.
  • Excellent interpersonal, communication, organizational, and computer skills; ability to work in a collaborative team.
  • Excellent written and verbal communication skills; bi-lingual helpful.
  • Proficiency in Outlook, Microsoft Office, OneDrive, and related tools.
  • Good judgment and problem-solving abilities; ability to work independently and in a team.
  • Ability to adhere to strict confidentiality standards.
  • Ability to work in a culturally diverse and inclusive environment.
