Shift Notes : 6 : 45P - 7 : 15A (12 Hour Nights) / No Call / EOW / 36 Hour Guarantee

Weekend Requirements : Every Other Weekend

On Call Requirements : No Call

Responsibilities : (50%)

  • Administers medications and performs treatments to assigned patient / residents and other technical skills within LPN license and training. Participates in team conferences and provides input to the Registered Nurse in the development of individualized nursing care plans. Reports changes in assessment, pertinent reactions and observations regarding patient / resident to the Registered Nurse and records these observations accurately and concisely. Assists with or institutes emergency measures for sudden adverse development in patients / residents.

  • Maintains the standards of nursing care and implements the policies and procedures of the hospital and Nursing Department, including maintaining the patients bill of rights / Long Term Care Resident Rights.

  • Accepts nursing care assignments and works as a member of the team. After initial assessment of the patient / resident by the Registered Nurse, continues to provide assessment of the patient / residents. Carries out appropriate nursing action as established by the Registered Nurse in the nursing plan of care. Participates in orientation of new personnel and performance evaluations of nursing staff.
