Position : Experienced Home Health Licensed Practical / Vocational Nurse (LPN / LVN)

Location : Allentown and surrounding area

Overview :

We are seeking a compassionate and experienced Licensed Practical / Vocational Nurse to join our home health care team. You’ll play a key role in delivering high-quality nursing care to patients in the comfort of their homes—focusing on independence, dignity, and holistic well-being.

Responsibilities

Conduct in-home nursing assessments and develop individualized care plans.

Administer skilled nursing procedures (wound care, medication administration, injections, catheter care, vital sign monitoring, etc.).

Monitor patient progress and communicate changes to the supervising RN or provider.

Educate patients and caregivers on disease management, medication adherence, and preventive health practices.

Provide emotional support and reassurance to patients and families.

Collaborate with physicians, therapists, social workers, and aides to ensure coordinated care.

Maintain accurate documentation within the electronic health record (EHR) system.

Uphold infection control, safety, and emergency protocols.

Participate in team meetings and case conferences to optimize patient outcomes.

Qualifications

Graduate of an accredited LPN / LVN program with current state license.

2–3 years of nursing experience (home health preferred).

Strong clinical skills in assessments, treatments, and patient care.

Excellent communication and interpersonal skills.

Proficient with EHR systems and technology.

Valid driver’s license, reliable transportation, and ability to travel within service area.

CPR certification (and other required state certifications).
