Duration : 13 weeks

Shift : MondayFriday, 85, with possible weekend rotation and on call

Job Description :

Sunbelt Staffing is seeking a skilled Home Health RN for a contract assignment in Bluffton, SC. This role involves managing a field caseload, completing OASIS documentation, and delivering compassionate care to patients in their homes. Nurses who are confident working independently and have strong assessment skills are encouraged to apply.

Key Responsibilities :

Conduct start-of-care assessments, routine visits, and discharges

Complete accurate and timely OASIS documentation in Homecare Homebase (HCHB)

Provide skilled interventions such as wound care, medication management, and IV therapy

Educate patients and caregivers on disease management and recovery plans

Coordinate care with physicians and interdisciplinary staff

Qualifications :

Active South Carolina RN license

At least 2 years of RN experience, including 1 year in home health

Proficiency in OASIS and Homecare Homebase (HCHB) required

Current BLS certification

Valid drivers license, reliable vehicle, and auto insurance

Able to meet all Joint Commission compliance requirements
