About The Position

Job Title : Masters Social Worker

Profession : Social Work

Specialty : -

Duration : Direct Hire

Shift : Mon-Fri

Hours per Shift : 8 : 00 AM - 4 : 30 PM

Experience : Prior experience with assigned patient population.

License : Licensed Social Worker (LSW / LCSW) required.

Certifications : Preferred - Licensed Clinical Social Worker (LCSW) or Licensed Practical Counselor (LPC).

Must-Have : Strong communication, advocacy, and problem-solving skills. Ability to work both independently and as part of a multidisciplinary team.

Description :

Provides skilled interventions for the support and resolution of patient and family crises.

Engages in problem-solving and decision-making.

Advocates and facilitates necessary support during life-changing events, such as adoption, guardianship, abuse, placement, and bereavement.

Mediates risk factors and assists patients in coping with disease or disability.

Addresses psychosocial and socioeconomic issues while adhering to departmental policies and procedures.

Receives referrals and conducts timely assessments for patients and families referred to social work service.

Integrates social work plans into overall patient care through interdisciplinary collaboration.

Delivers comprehensive case management, discharge planning, continuing care services, advocacy, and clinical social work services, including crisis intervention and education.

Provides skilled services in areas related to abuse and neglect, advanced directives, psychiatric placements, and end-of-life issues.

Utilizes hospital and community-based resources and entitlements effectively.

Refers patients and families to appropriate services to ensure continuity and quality of care.

Develops specialized knowledge of resources for specific patient populations.

Facilitates efforts to obtain insurance coverage for services.

Documents patient care plans and interventions promptly and completely in medical records.

Completes required statistical reports.

Collaborates with interdisciplinary teams to develop complex discharge plans and conduct transitional planning for patients moving to various facilities.

Encourages continuous professional development through supervision and attendance at relevant conferences and training.

Participates in community education and support group activities as approved.

Exhibits independent judgment in decision-making from diversified alternatives.

Coordinates activities of others and works directly with patients and customers.

Engages with healthcare professionals and staff, frequently interacting with individuals at the Director level or above.
