Job descriptionProvide physical therapy services in skilled nursing facility setting Conduct patient assessments and develop individualized treatment plans Implement therapeutic interventions and rehabilitation programs Document patient progress and treatment outcomes Collaborate with interdisciplinary healthcare team members Educate patients and families on treatment plans and home exercise programs Maintain current physical therapy licensure and certifications Follow facility policies and procedures for patient care Participate in care plan meetings and discharge planning Ensure compliance with regulatory requirements and standards Work day shift hours from 8 : 00 AM to 4 : 30 PM Provide quality rehabilitative care to geriatric population Active, unencumbered license in NH Minimum of 1-7 years of relevant experience, depending on specialty Current Certifications : BLS and / or ACLS, plus any specialty-specific requirements Experience with EMR systems (Epic, Cerner, as applicable) Current Resume listing detailed work history (MM / YYYY format) Two current clinical references State-required professional license Physicial exam (completed within the past 12 months) TB skin test or Quantiferon Gold (within the past 12 months) Immunity Titers - MMR / Hep B / Varicella (declinations may be accepted) Current Tdap and Flu vaccinations Must pass background check and drug screening A modern, mobile-first platform built for how you want to work Clear, upfront pay rates - no surprises Book contracts and shifts on your schedule, 24 / 7 Maximize your take-home with top tier, tax-free housing & travel stipends Contract extension bonuses Referral rewards Optional retirement benefits Life insurance Retention bonus 401k retirement plan Referral bonus Sick pay Holiday Pay
