Job Overview – Family Medicine Physician (Outpatient) :

Compensation : $250,000 – $275,000 / year

Location : Brooklyn, NY

Schedule : Monday to Friday, 9 : 00 AM to 5 : 00 PM (with rotating Saturdays)

Join our client’s community health team as a Family Medicine Physician (Outpatient) in Brooklyn, NY, providing full-scope primary care to patients of all ages in a diverse, underserved setting. In this outpatient role, you’ll conduct comprehensive evaluations, develop treatment plans, coordinate specialty referrals, and deliver preventive care to pediatric, adolescent, and adult populations. Ideal candidates are board-certified physicians committed to health equity, social determinants of health (SDOH), and advancing care in community-based environments.

Responsibilities as the Family Medicine Physician (Outpatient) :

• Primary Care Delivery : Provide comprehensive care for patients of all ages, including exams, diagnostics, and treatment planning.
• Preventive Health : Administer routine screenings, immunizations, and patient education based on individual needs.
• Care Coordination : Collaborate with specialists, behavioral health, and support teams to ensure integrated care.
• Diagnostics & Treatment : Order and interpret labs and imaging, develop diagnoses, and initiate medical interventions.
• Documentation & Quality : Maintain accurate EMR records and meet compliance, productivity, and quality standards.
• Community-Based Care : Deliver culturally sensitive care while addressing challenges related to social determinants of health.
