Weight Coordinator (GNA) At FutureCare
At FutureCare our Weight Coordinator (GNA) will play a pivotal role in monitoring and managing residents overall nutritional and hydration status. The Weight Coordinator is responsible for obtaining weights as required by facility protocol. This position requires daily communication with the facilities RD / DON and / or Administrator.
Salary Range - $17-$19 an hour

The salary mentioned above reflects the potential base pay range for this role. Bonuses or other incentives (if applicable) are offered separately. Offers will consider such factors as overall experience, job-related qualifications, location, certifications / training, etc.
Responsibilities
- Obtain weekly weights completed by the end of day every Wednesday
- Obtain weights on all admissions / re-admissions within 3 days of (re) admission to ensure accuracy of admission weight
- Attend weekly nutrition committee meetings and participate in discussion
- Notify DON, Administrator of any problems related to scales and weighing residents
- Assist with dining room services Monday-Friday, document meal percentage intake and communicate to nursing units
- Communicate daily with the RD any weight / nutritional concerns observed
- Perform re-weighs when weekly weight is greater or less than 5# compared with previous weeks weight; when monthly weight is greater or less than 5# compared with previous weight; or if a resident is less than 100# and weight is greater or less than 3# from previous weight
- Identifies self-learning needs related to job description
- Seek out appropriate resources to meet learning needs
- Attends twelve (12) hours per year of Nursing Assistant / Nursing related workshops or seminars including all mandatory educational classes such as Infection Control, Fire and Electrical Safety
- Set and implement goals and policies consistent with the philosophy and purpose of the facility
- Abide by the standards identified in FutureCare's Statement of Corporate Ethics and the Corporate Compliance Plan
Qualifications
Graduate of state approved Geriatric Nursing Assistant programMust possess a current active GNA certification for the state of MarylandDemonstrates positive and effective communication skillsResponds to resident's / families' requests for services in a courteous manner with no reported variancesAbility to provide accurate information regarding resident condition to assigned nurse in a timely manner appropriatelyHandle confidential information with discretion without documented variancesCommunicate pertinent information and requests through established organizational channelsInteracts with others in a considerate, helpful and courteous mannerResents neat appearance in appropriate dress and identification as required by the positionEqual Opportunity Employer
