Doctor of Pharmacy (PharmD) degree, active Pharmacist license, and minimum 1+ years Pharmacist experience required. Applicants who do not meet these qualifications will not be considered.

We are seeking an experienced Oncology Clinical Pharmacist to join a hospital-based team in Delaware. This is a full-time, direct-hire opportunity on a dayshift schedule.

Position Overview :

The Oncology Clinical Pharmacy Specialist will support cancer patients across their treatment journey by ensuring accuracy of treatment plans, optimizing supportive care measures, and serving as a clinical resource for oncology fellows, residents, and staff. This role is integral to advancing high-quality, evidence-based oncology care.

Key Responsibilities :

Establish collaborative practice programs with oncologists, including supportive care for patients receiving chemotherapy in both inpatient and outpatient settings.

Conduct educational sessions for pharmacy, nursing, oncology staff, and other healthcare professionals.

Serve as a subject matter expert in oncology by addressing drug information requests and reviewing care plans to ensure alignment with national guidelines.

Participate in rounds with oncology fellows, nurse practitioners, and physicians to provide remendations on medications, dose adjustments, and supportive care.

Provide patient-specific medication counseling for oral and intravenous chemotherapy.

Review, update, and maintain oncology care plans within the Electronic Health Record.

Identify and resolve medication-related problems based on clinical presentation and laboratory data.

Serve as a preceptor for pharmacy residents and students.

Contribute to oncology service line initiatives focused on safety, quality, and cost-effective drug therapy.

Qualifications : PharmD required.

pletion of PGY1 residency with at least 3 years of clinical pharmacy practice OR PGY2 residency in oncology OR 3+ years of oncology practice.

Current pharmacist license required.

Board Certification in Oncology Pharmacy (BCOP) required.

Minimum 3 years of hospital experience with strong clinical oncology training.

Preferred Qualifications :

PGY2 ASHP-accredited residency in oncology or related specialty.

PGY1 ASHP-accredited residency plus 3+ years of oncology experience and BCOP.

5+ years of oncology pharmacy practice with BCOP.
