Job Description

Job Description
About This Role
As a Provider Engagement Specialist, you will have the exciting opportunity to build meaningful relationships with healthcare providers who share our passion for caring for elderly participants in the PACE program. Your work will play a vital role in ensuring that our participants receive the highest quality care through strong partnerships and seamless collaboration.
You will serve as a trusted liaison between our internal team and the provider community, ensuring open lines of communication, addressing any concerns, and helping to improve processes that make care delivery even more effective. By facilitating outreach events, training sessions, and meetings, you’ll help foster an environment where providers feel valued, informed, and connected to our mission.
This role is ideal for someone who loves working with people, thrives in building relationships, and enjoys making a positive difference in the lives of seniors. Your efforts will directly support the well-being of our participants and contribute to the overall success of our PACE program.
What does success look like as a Provider Engagement Specialist
  • Build Meaningful Relationships: You’ll develop and maintain strong, trusting connections with primary care physicians, specialists, and healthcare providers who care for our PACE participants.
  • Foster Open Communication: Coordinate and lead meetings, outreach events, and training sessions to keep providers engaged and up-to-date with our PACE program offerings, processes, and expectations.
  • Collaborate as a Team: Partner with our clinical, administrative, and operations teams to enhance provider participation and ensure smooth coordination of care.
  • Solve Problems Proactively: Serve as a go-to resource for providers, helping to address their questions, resolve concerns, and ensure clear communication with our PACE interdisciplinary team.
  • Drive Process Improvements: Look for opportunities to improve the way we collaborate, making sure that we’re always working towards the best possible outcomes for our participants.
  • Monitor and Support Quality: Work with the quality team to track provider performance metrics and collaborate on areas for improvement.
  • Stay Informed and Adapt: Stay on top of industry best practices and regulatory changes that may impact provider engagement, and help the team adapt when needed.
  • Resolve Issues with Care: Assist in resolving provider-related challenges in a way that strengthens relationships and supports smooth, high-quality care.
  • Support Growth Efforts: Help with strategies for expanding our network and recruiting new providers as needed.
  • Pitch In as Needed: You’ll also be flexible, contributing to other duties and tasks that help ensure the success of the PACE program.
What does an ideal candidate look like?
  • Education & Experience: You have a Bachelor’s degree in Healthcare Administration, Business, or a related field, and bring at least 3 years of experience in provider relations, healthcare administration, or a similar role within a healthcare setting.
  • Specialized Knowledge: Familiarity with PACE or geriatric care models is highly preferred, so if you have experience in this space, you’ll be a great fit for this role!
  • Interpersonal Skills: Your ability to build rapport with a variety of healthcare providers makes you a natural at creating lasting, positive relationships. You’re comfortable working with diverse professionals and thrive in collaboration.
  • Communication: You’re an effective communicator who can clearly and compassionately convey information in person, over the phone, and in writing.
  • Organization & Attention to Detail: You have excellent organizational skills and a keen attention to detail, ensuring that nothing falls through the cracks when coordinating with our providers.
  • Teamwork & Independence: While you enjoy working as part of a team, you’re equally capable of working independently to drive success and meet your goals.
  • Tech-Savvy: You’re proficient in Microsoft Office Suite and have experience using healthcare management software, helping you stay organized and on top of your tasks.
  • Licensing & Transportation: As this role requires travel to meet with providers, candidates must have a valid CA driver’s license, reliable personal transportation, a good driving record, and auto insurance as required by state law.
What are the Physical Requirements?
  • Work where it matters: You’ll spend time at your laptop and in virtual meetings, so you’re comfortable sitting and working on a computer/phone for extended periods.
  • Out in the community: You’ll also be on your feet during provider visits, partner meetings, and community events, with the ability to stand and walk for extended periods.
  • Local travel is part of the role: You can drive throughout our service area, sometimes with multiple stops in a day (valid driver’s license, reliable transportation, and proof of insurance may be required).
  • Light lifting and set-up: From time to time, you’ll lift/carry materials like brochures or small event supplies—up to 25 pounds.
  • Hands-on coordination: You’re able to reach, bend, and use your hands/fingers to handle materials, set up presentations, and use standard office equipment.
  • Confident communicator: You can communicate clearly and professionally in person, by phone, and via video—especially when presenting to partners and collaborating with internal teams.
  • ADA statement: We will provide reasonable accommodations to support individuals with disabilities in performing the essential functions of the role.
Where You'll Work:
  • Hybrid and community-facing: This role blends time in the community with focused work from home and/or onsite at a PACE center/office.
  • Out in the community: You’ll regularly meet with providers and partners across the service area, which may include time outdoors and exposure to changing weather while traveling between locations or supporting events.
  • Relationship-driven and fast-paced: You’ll juggle multiple priorities, partner needs, and deadlines—success here comes from strong organization, responsiveness, and follow-through.
  • Highly collaborative: You’ll partner closely with Growth, Enrollment, Clinical/IDT, Center Leadership, and Compliance to ensure engagement efforts are aligned, coordinated, and compliant.
  • Tools of the trade: Expect routine use of CRM and reporting tools, email, phone, and video conferencing to track outreach, document activity, and share updates.
  • Flexibility when it counts: You’re open to occasional evenings or weekends for important partner events or community activities.
