Manager of Habilitation and Activities is responsible for understanding rehabilitation and habilitation, as well as the social, recreational and wellness needs of frail and culturally diverse elders. Ensures that the Activities Staff produce high caliber, innovative, stimulating and appealing programming. Supports, directs, mentors and coaches the Activities staff across PACE sites. Works independently under the supervision of the Director of Rehabilitation. This position is full time M-F 8am to 4pm.
ESSENTIAL RESPONSIBILITIES :
- Manages the planning, development and implementation of programs and activities that will support habilitation, wellness and recreation at all of the Adult Day Health Sites and in the home setting.
- Manages the planning, development and implementation of programs and activities that will support habilitation, wellness and recreation through telehealth and virtual activities.
- Works closely with the activity staff to ensure development and delivery of high quality activities.
- Recruits, develops and motivates direct reports. Holds staff accountable for the range of assigned duties. Initiates and communicates a variety of personnel actions including employment, termination, performance reviews, salary reviews and disciplinary actions.
- Supports and promotes participation in virtual activities.
- Leads staff in community outreach and delivery of in-person activities in the home.
- Develops a way to monitor and improve participant satisfaction.
- Remains current with activity and recreation trends and resources, providing in-service education to Activity staff across the Centers.
- Reviews need for updates to departmental assessments and progress notes in the EMR.
- Organizes, prepares and implements policies and establishes goals, as directed.
- Coordinates with IDT and Center Managers as needed.
- Participates in Element Care Committees, as required, and communicates relative information, as necessary.
- Participates in Quality Improvement projects and assessments, as needed.
- Coordinates community outreach for activities related events.
- Performs all duties within the Occupational Therapist scope of practice.
- Ability to pass a fit test. Position requires mask where seal is critical. Incumbent is required to not have facial hair that interferes with a tight seal of the respirator.
- Frequent local travel.
- Provides coverage for absent staff, as needed.
- Performs other duties as required.
