NPORT is seeking a dedicated and compassionate Physical Therapist (PT) to join the rehabilitation team at a respected Rehabilitation & Nursing Center located in Amityville, NY.

This position offers a rewarding opportunity to work with a multidisciplinary care team committed to improving patients’ mobility, independence, and quality of life.

Responsibilities :

• Conduct comprehensive physical therapy evaluations and develop individualized treatment plans.
• Provide direct patient care, including therapeutic exercise, balance training, gait training, and functional mobility interventions.
• Collaborate closely with physicians, nurses, occupational therapists, and speech therapists to ensure coordinated high-quality care
• Document all patient care activities accurately and in a timely manner in accordance with facility regulatory standards.
• Educate patients, families, and caregivers on therapy goals, techniques, and home exercise programs.
• Participate in discharge planning and ensure smooth transition of care.
