Position Summary
As the HIV Care Coordinator, you will be a key member of the HIV Care Team. You will coordinate and deliver primary care services to patients living with HIV, manage individualized care plans, provide patient education, assist in triage, and help connect patients with needed resources. This role also includes supporting HIV testing efforts and offering spiritual care to those in need.
Core Responsibilities
Coordinate and deliver primary care services for patients living with HIV
Develop and maintain individualized care plans based on ongoing assessments
Triage and assess patients who call or walk in for care
Provide patient education and empower patients to manage their health
Collaborate with medical providers and staff to ensure holistic care
Assist with HIV testing and follow-up care
Provide spiritual care and support as needed
Uphold Esperanza’s mission and demonstrate Christlike compassion in all interactions

Qualifications & Experience
Licensed Registered Nurse (RN) in the state of Pennsylvania
Experience in hospital or community health settings
Previous experience with HIV / AIDS care preferred (training available)
Understanding of HIV / AIDS-related care standards
Strong interpersonal and communication skills
Ability to work independently and within a team
Bilingual in English and Spanish preferred (Spanish immersion training available)
