Nurse (RN) Case Manager

They collaborate with healthcare providers, monitor patient progress, and ensure a smooth transition between different stages of care well advocating for the patients' well-being.\n\nWhat you will do : \n

• As a Registered Nurse RN Case Manager Department, you will serve as a driver of a collaborative, process which coordinates, monitors, and plans health care services throughout the patients' illness.\n\nMinimum Qualifications : \n
• Education : Bachelors of Science in Nursing BSN degree\n
• Licensure : Current Licensure as a registered nurse RN in Connecticut\n
• Experience : Five years of diversified, progressive experience in acute care and / or other settings within the continuum of Care including an understanding of nationally recognized medical utilization criteria, HCFA / OBRA and state of Connecticut regulatory compliance issues, and JCAHO accreditation accountabilities.\n
