Summary :
Utilizes the nursing process and evidence-based practice to provide patient-driven, holistic care with the collaboration of the Core Team (Patient, Primary Care Physician, RN Care Manager, Therapy and aide) and Expanded Team (family / caregiver and internal and community-based services involved in providing care to the patient). Develops care protocols that support care for a panel of patients throughout the continuum of care to assure that care is timely, appropriate, of high quality and cost effective. Utilizes health education and telecommunication as applicable to promote therapeutic relationships and shared decision-making between the patient and the interdisciplinary team to support patient self-management, prevention and wellness. Demonstrates leadership in the delivery and improving holistic care through collaborative strategies with others. Employs performance improvement strategies to reach quality and cost effectiveness goals.
Palliative Care RN Program Coordinator is an exciting opportunity to work with an interdisciplinary team to provide high quality Palliative Care services to patients and caregivers with chronic and terminal illness. This role requires strong organizational skills, attention to detail and a collaborative approach to both direct patient interaction and administrative work.
Minimum Qualifications :
Formal Education Requirements :
a. Bachelor’s of Science in Nursing (BSN)
b. Graduate of a program of professional nursing that is accredited by the Commission on Collegiate Nursing Education (CCNE) or Accreditation Commission for Education in Nursing (ACEN)
c. Current license to practice registered nursing in the State of Ohio
