Franciscan Village , a part of Franciscan Ministries, is a vibrant senior living community located in Lemont, Illinois. We are a continuing care retirement community (CCRC), offering a variety of senior lifestyle options including independent living, memory support assisted living, and skilled nursing / long-term care. Additionally, the onsite rehabilitation clinic offers inpatient and outpatient physical, occupational, and speech therapies, and state-of-the-art programs designed to enhance the quality of life for residents and patients in the local community. Position Description :

Ideal candidate must have effective communication skills for interacting with physicians, families and staff and residents

Recent experience in a skilled environment as a wound care nurse with in depth knowledge of treatment protocols, measuring wounds, documentation, etc. Good assessment skills and communication skills. They currently have an opening at one of their 127 bed skilled nursing facilities. This is a wonderful opportunity for an experienced Wound Care Coordinator.

Wound Care- LPN or RN

Licensed Nurse in good standing

Experience in a wound care nurse role, preferably management

Ability to care for high acuity patients

Ability to facilitate teamwork

Valid CPR / BLS

Ability to effectively prioritize in a fast pace environment

Wound Care Certification preferred

Ability to work in Acuity environment and alongside a team

3-5 years experience in skilled nursing

Management Experience

Good Communication Skills

PCC

On Call Rotation

Proven Results in Wound Care Plan implementation resulting in a decrease of wounds for one or more patients over a reasonable length of time

Experience working in skilled nursing facility a plus

Knowledge of regulations set forth through Illinois Department of Public Health

Oversees the proper administration of total care to our residents. Works collaboratively with members of the healthcare team. Maintains standards of professional nursing practice.
