Summary : As a Nurse Practitioner, you will deliver high-quality, compassionate care to a diverse
patient population. You will work closely with physicians, multidisciplinary professionals, and
community agencies to ensure continuity of care. Your role is critical in promoting health, preventing
disease, and managing the ongoing healthcare needs of the community, including pediatrics, family
practice, and psychiatry based on your specialty.
Essential Duties and Responsibilities :
Conduct comprehensive health assessments, including obtaining health histories, performing
physical exams, and ordering diagnostic tests to develop individualized care plans.
Provide continuous and coordinated primary healthcare by establishing mutually set goals and
delivering care in collaboration with patients, their families, and other community care teams.
Prescribe and manage treatment modalities, including medication and therapy, to address the
health and illness needs of patients.
Ensure accurate and timely documentation in the Electronic Health Record (EHR) system
according to organizational standards.
Collaborate with the care team to conduct daily huddles, ensuring seamless communication
and care coordination. Manage a patient panel of up to 1,200 unique patients annually,
fostering strong patient-provider relationships. Participate in quality improvement initiatives,
including peer reviews and clinical protocol development, to enhance patient outcomes and
clinical standards.
Serve as a mentor and supervisor to advanced practice nursing students, helping to shape the
future generation of healthcare providers.
Ensure adherence to legal and ethical guidelines as established by the American Medical
Association, the State of Illinois Department of Professional Regulation, and specialty-specific
practice standards.
Stay current with clinical best practices through continued professional development,
participation in regular team meetings, and completion of required education and
certifications.
Qualifications : To perform this job successfully, an individual must be able to perform each essential

Active Illinois Registered Nurse licensure required.
Bachelor’s degree in nursing or Master’s / Doctorate in Advanced Practice Nursing from an
accredited institution with a Nurse Practitioner specialty.
Certification in a specialty area (e.g., pediatrics, family practice, psychiatry) required.
1-3 years of experience as a Nurse Practitioner preferred.
Bilingual skills (Spanish or another language) highly desirable.
Ability to work collaboratively in a team-based care model and provide culturally sensitive care
to diverse populations.
Strong commitment to serving medically underserved communities and promoting healthcare
access and equity.
Salary and Benefits :
Compensation : The annual salary range for this position is $100,000.00 and $135,000.00, based
on experience and qualifications.
