Job Description

The Care Coordinator serves as a point of contact for patients, families, and healthcare providers to ensure seamless, patient-centered care. This role involves communicating with patients regarding next steps in their treatment and care plans. The Care Coordinator meets with patients after they meet with their providers to help schedule follow-up appointments and discuss additional resources available through the clinic and the county. Additionally, the Care Coordinator will verify that required steps for additional care resources are being taken.

Responsibilities

• Communicate with patients regarding their treatment and care plans.
• Assist patients in scheduling follow-up appointments.
• Discuss and provide information about additional resources available through the clinic and the county.
• Ensure that required steps for additional care resources are being taken.

Work Environment

This position is based on-site at a community health clinic, working Monday through Friday during clinic hours. The work environment offers an excellent opportunity to gain experience supporting a Care Coordination team, which can be challenging to acquire for lower-level healthcare employees.

Job Type & Location

This is a Contract position based out of Minneapolis, MN.

Pay and Benefits

The pay range for this position is $21.00 - $31.00 / hr.
