Description : ASAP Needed!

8 : 00 AM - 4 : 40 PM, Monday - Friday.

No weekends or on-call responsibilities.

The RN Care Manager will oversee a daily caseload of patients on the inpatient units with a variety of care needs.

Will attend interdisciplinary rounds.

Complete initial assessments.

Facilitates the development of a plan of care in collaboration with the patient, family, caregiver, and the healthcare team.

Communicate consistently with medical teams and patients regarding the progress of care needs related to post-discharge care, such as home health and inpatient rehabilitation.

Will interact with post-acute care referral sources to establish a plan.

Document and place orders for home health, durable medical equipment, and nutritional needs within the electronic medical record system.
