Registered Nurse - Case Manager - Community Hospice

As a vital member of our interdisciplinary team, you’ll deliver skilled nursing care, educate patients and families, and create a supportive environment that honors their wishes.

Position Specific Requirements :

• 2 years' experience in either home health, home hospice, or med surg - or a combination of these three settings.

On-call : Not typically, variable dependent upon program needs.

Incentives : $10,000 Sign On Bonus for full time external applicants, prorated for part time roles! Additional terms and conditions apply.
