Job description

Haven Home Health is excited to hire TWO Physical Therapists (FT or PT) to join our team in the Palm Beach Gardens area! We are looking for motivated and compassionate therapists who are passionate about delivering high-quality care to patients in the comfort of their homes.

Coverage Areas

  • West of Royal Palm Beach / Loxahatchee
  • Palm Beach Island / Palm Beach Gardens / Jupiter / Riviera Beach

Responsibilities

  • Provide skilled physical therapy services to patients in their homes
  • Perform evaluations, develop care plans, and provide treatments based on patient needs
  • Educate patients and caregivers on safe home exercise programs
  • Document patient care and progress according to agency and regulatory standards
  • Communicate effectively with the home health team to ensure quality care

    • Requirements

  • Licensed Physical Therapist (PT) in the state of Florida
  • Home Health experience preferred (minimum 1 year), but we are willing to train the right candidate
  • Strong communication and interpersonal skills
  • Ability to work independently and manage a patient caseload
  • Valid driver's license and reliable transportation
