Provide skilled nursing care in patients’ homes

Develop individualized care plans and manage symptoms

Educate and support patients and families

Collaborate with physicians, social workers, and other care team members

Maintain accurate documentation

Qualifications

Active RN license in Arizona (or compact-eligible)

Minimum 1 year of clinical RN experience (hospice / home health preferred)

Strong communication and critical-thinking skills

Compassionate, adaptable, and patient-focused

Valid driver’s license and reliable vehicle
