Advanced Practice Provider (APP) In-Home Chronic Care

About the Role:
We re seeking Advanced Practice Providers to join a nationally recognized leader in in-home care for patients with chronic kidney disease and end-stage renal disease. This innovative model combines technology-enabled care management, medication review, and patient education to improve outcomes, reduce costs, and expand access to high-quality, evidence-based care. A Day in the Life:

• Conduct comprehensive initial assessments, follow-ups, and hospital transitions
• Perform hands-on, head-to-toe assessments and obtain full vitals
• Manage medications and provide disease process education
• See approximately 4 5 patients per day
• Visit durations: 90 minutes for new patients, minutes for follow-ups
