Location : North area - Boulder, Superior, Erie, Louisville & surrounding areas

Assignments are typically grouped based on your residential location for convenience.

Key Responsibilities

  • Conduct personalized in-home evaluations and assessments.
  • Develop individualized treatment plans and maintain accurate medical records.
  • Collaborate with physicians, staff, and external services for effective care coordination.
  • Monitor patient care quality and ensure treatment plan implementation.
  • Review caseload documentation regularly to track progress and treatment effectiveness.
